Supervision Contract

We have decided to enter into a supervisory relationship together. We have reviewed several issues to help us establish an agreed-upon timeframe for this experience. This contract aims to outline those issues and serve as a resource for our expectations.

Supervisors’ Background & Philosophy 

Philosophy: As a therapist, my approach is drawn mainly from Jungian, Systemic, and Emotionally Focused Couples Therapy. We will assess your cases and your growth as a clinician by examining issues related to the self-as-therapist and the therapeutic frame. Participating actively in supervision can have a significant impact on both your professional and personal life, accompanied by the risks and benefits that come with change.

Background: I have been a licensed marriage and family therapist since 2019. I earned my B.S. in Human Services Counseling in 2006, M.S. in Marriage, Family, and Child Therapy in 2016, and my PhD in Sex Therapy in 2025. I have only practiced in private practice since 2017. I’m an AAMFT-approved supervisor and have been supervising since 2020. I am a Certified Sex Addiction Therapist through IITAP and a Certified Sex Therapist through AASECT.  

Logistics 

We have agreed to commit to the following timeline for this supervision contract: commencing on the date signed on this form and lasting until the individual is fully licensed by the State of Nevada.

Primary State Intern Supervision: No less than seven supervision contact hours per calendar month. At least one of these contact hours must be an individual (one-on-one) session. Supervisors or trainees may decide to increase the number of monthly supervision hours if either deems an increase is needed. Fees: $250 A Month

Secondary State Intern Supervision: There is a minimum of six supervision contact hours per calendar month. Fees: $175 A Month ($75 for each  individual supervision session)

Each month, the supervisee will schedule which supervision they will attend and pay for ALL supervision at the start of the month. (No later than the 15th). Any payment after the 15th will accrue a $25 late fee for every five days past the due date. (Example 15-20th – $25, 20th – 25th – $50). Failure to pay will constitute a disillusion of supervision, and I will not sign any documents until the balance is paid in full.

To meet the primary supervision requirement of 160 hours within two years, a supervisee must meet with their primary supervisor 6.5 hours each month. To meet the 300 hr minimum within two years, a supervisee needs to have a trend of 13 hr supervision per month (primary and secondary) 

At any time, I can request that you review any documentation, including PARS, session notes, videos of sessions, and any other relevant materials to evaluate your development. The trainee may not sign any documentation without express written permission from the supervisor. 

During monthly one-on-one supervision, the trainee will be given the opportunity to provide feedback to the supervisor. This one-on-one session will review supervision goals, evaluate development, and address ethical, legal, or impersonal issues. Any disputes, allegations of unethical behavior, threats of a complaint or lawsuit, or personal disagreements with peers, colleagues, or supervisors must be discussed during individual supervision.

Stumbling Blocks

We will first attempt to resolve any stumbling blocks, conflicts, or adjustments to expectations among ourselves. Suppose we cannot work through the issue. In that case, another AAMFT-approved supervisor will be asked to assist us in resolving any of the above-listed stumbling blocks. If the problem remains, it will be presented to the BOE for direction.

Should we encounter an issue related to break-in ethics or laws, including safety concerns or mistreatment of a client, or the impact of our profession on the broader community, a plan of action to address it will be developed. Please be advised that, at a minimum, any issue or incident of this nature will also be shared with the BOE. If no resolution or if other problems stated above occur, supervision may be terminated.

Supervision Contract Disillusion

This supervision contract will end (a) once you have received your independent license from the NV BOE (primary or secondary) or (b) you have completed your minimum commitment for supervision hours (secondary) (c) at any point during supervision either myself (supervisor) or you (supervisee) can decide to end the relationship. 

In the case of disillusion, for instance (c), we commit to the following process. Discuss disillusion with the supervisor/supervisee during a one-on-one supervision session before reaching out to any potential new supervisor. Then the supervisee & supervisor will determine a timeline for disillusion. The supervisee must continue attending supervision by maintaining at least minimal contracted hours during the transition. 

Methods of Supervision

Supervision is an activity in which you must bring cases, issues, and areas of growth. Supervision can/will include group supervision, individual supervision, and tele-supervision. Case presentations can consist of live viewing and recording, while following all steps to maintain confidentiality. Supervision conversations are scheduled in advance, except for Emergency Protocols.

Social Media, Business Development, and Professionalism

Personal or professional social media use must be used ethically and responsibly by trainees. All trainees must have a social media policy, which can be provided to you. Any use of blogs, vlogs, podcasts, or social media that represents you must be shared and discussed on an ongoing basis. Part of supervision is developing your professional skills and growing your business skills, which can be addressed in individual supervision.

Ethics & Reporting 

Trainees agree to follow the AAMFT Code of Ethics and any additional ethical codes related to their training and professional identity. Any ethical disputes must be addressed with the supervisor. 

Mandated reporting: Any mandated reporting issues must be addressed with the supervisor within 12 hours. Upon determining if a report is necessary, all made reporting must be reported within 24 hours of discovery.  

Confidentiality 

Case content shared during supervision is confidential information and will be held as such. The supervisor relationship itself is a public record and does not constitute a confidential relationship. However, personal and case information will be held to the highest standards of confidentiality. As your supervisor, I strive to engender professional and personal growth while remaining safe, supportive and affirming of each trainee’s developmental growth as a professional therapist. 

Emergency Protocols 

For primary interns or student interns, please follow the steps below. Secondary intern, please reach out to your primary first. 

Any mandating reporting (or questions around reporting)  of ethical issues,  personal issues that interfere with your ability to perform therapy

  1. If you think you, a client, or another individual is in imminent life-threatening danger, first call the police. (if not in imminent danger, start on the second step) 
  2. Do the following:
    1. Text the following – “Emergency Consult Need” – Name the topic area (ethical issues, reportable issues, personal issues) 
    2. Call and leave a message stating the same as you did in text. 
    3. During the hours of 8 am to 8 pm, we will respond within 2 hours.
  3. If I do not return your text/call within 2 hours, reach out to your secondary supervisors or consult with a licensed therapist until I get in touch with you. 

You will be notified via email when I will be unavailable for contact. You will need to contact your secondary supervisor for any emergency issues during this time. 

Supplemental Requirements 

As in life, in therapy, and supervision, many unknowns arise. To address any unknown part, both the trainee and the supervisor should develop a plan of action that may include, but is not limited to, personal therapy, additional reading, training, or increased supervision hours. 

Limits of Supervision  

This contract serves as an agreed-upon framework and general outline of the supervisory relationship’s expectations. It is not an exhaustive list, and it can be reviewed and revised by either you or me at any time. If a revision or edit to our contract is needed, we cannot agree. Our supervisory relationship will follow the guidelines outlined in our last agreed-upon contract until a Board-approved termination of our supervisory relationship is recognized. 

Miscellaneous

  1. Approval from me as a supervisor is needed for work at any agency. I must have a working relationship with any agency you wish to be employed by, to ensure that you are fully protected and are practicing ethically.
  2. The use of social media is strongly encouraged to promote a practice, working with myself as a supervisor to ensure ethical representation, along with the use of a social media policy, is critical.
  3. The State intern will not accept or participate in any consultation calls with any potential clients.

Identification of Goals for Supervision

We have identified the following objectives in our collaboration. These goals will be updated and revised regularly during the evaluation process. 

  1. Growth of the therapist (understanding the significant tenets of transference and its usefulness in the therapeutic encounter, development of a professional theoretical model, a clear understanding of the AAMFT’s Codes of Ethics, as well as the adoption of an ethical decision-making model, a clear understanding of Nevada state law (NRS), self-care methods, and continued education).
  2. Growth as a counseling professional within the greater professional counseling community. 
  3. Complete all the various required hours for licensure, including completing the required examination for your state. 

Evaluations 

Regularly during your time in supervision, we will be using an evaluation tool essential for your development. The evaluation device we will use will be given to you with this supervision contract. The evaluation device is based on Nelson & Johnson’s Basic Skills Evaluation Device. Quarterly evaluation will allow us to personalize your development within the field.
